Paediatrician asked for her last weight and neither of us could find it

Daughter is 7 months. We set up a shared folder after she was born. It has four files in it, all from the first month. Since then the vaccination card has gone into a drawer somewhere, the doctor's notes are photos on my partner's phone and last month's lab report is in a chat I can't find. Today the doctor asked for her last weight and we both stood there scrolling. What keeps a system like this going past the first month?

for the weight i just started asking the clinic to write it on the vaccination card at every visit. the card goes to every appointment anyway. everything else in our house is still a mess seven years on

We keep one physical file that goes in the bag to every appointment, no exceptions. Whoever attends photographs any new page that same day into a shared album both of us can see. The original vaccination card lives in the file, never in a drawer. The rule that matters is "same day"; anything left for later gets lost.

Do you have one vaccination card or two? We got one from the hospital and another from the paediatrician and nearly took the wrong one to an appointment.
