Daycare wants our 5 month old's 'daily schedule' on a form. She doesn't have one.

Daycare starts in July. The form has neat boxes: wake time, feed times, nap times, nap duration, bedtime. She wakes between 5 and 7. Feeds when she's hungry. Naps when she naps, anywhere from twenty minutes to two hours. Bedtime is a range. Do I write the ranges? Make up something tidy? Or is the form really asking whether we plan to build a routine before July? We'd like to. She hasn't agreed.
